Health Insurance For Senior Citizens

Ayushman Bharat now gives every Indian aged 70+ a free Rs 5 lakh health cover, no income check needed. Here's how to find PMJAY hospitals near you

Health Insurance: Every Indian citizen aged 70 and above is now entitled to free hospital treatment worth up to Rs 5 lakh a year under the Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB PM-JAY), through a dedicated add-on called the Ayushman Vaya Vandana scheme. The cover applies irrespective of income or socio-economic status, marking a departure from the original PMJAY design, which was targeted at economically weaker families identified under the 2011 Socio-Economic Caste Census.

For seniors who are already part of a PMJAY-covered family, the Rs 5 lakh under Vaya Vandana comes as an additional, separate top-up — not merged with the family’s existing cover. That distinction matters for households where multiple generations are insured under the scheme, since it means a 70-plus parent effectively gets their own individual cushion on top of the family’s cover.

Health Insurance: How Many Hospitals Are Actually Empanelled

According to a government reply in the Rajya Sabha, more than 27,000 hospitals — including nearly 11,908 private hospitals — are currently empanelled under AB PM-JAY nationally, offering a combined 13.27 lakh beds. Separately, the health ministry has told the Lok Sabha that roughly 14 lakh Vaya Vandana cards had been issued to senior citizens by late November, against an estimated eligible base of 6 crore individuals across 4.5 crore families — suggesting a large gap between eligibility and actual enrolment that the government is still working to close.

Because empanelment numbers shift regularly as new hospitals join or exit the network, there’s no single static list that stays accurate for long. The reliable way to check is directly through the government’s live database rather than a downloaded PDF, which can go stale within weeks.

The National Health Authority maintains a searchable hospital directory at hospitals.pmjay.gov.in, where beneficiaries can filter results by state, district, and medical speciality before heading to a hospital. The same information is also accessible through the Ayushman Bharat app. All public hospitals in states implementing the scheme are automatically deemed empanelled, while private hospitals are empanelled individually by each state’s health agency after meeting quality and service benchmarks — which is why the private hospital list looks different from state to state.

Treatment under empanelled hospitals is meant to be fully cashless and paperless, with no facilitation fee payable to any agent or middleman for using the card.

Health Insurance: Who Is Eligible and How to Apply

Any Indian citizen aged 70 or above can apply, with no income test involved. Eligible seniors can generate their Ayushman Vaya Vandana card online at beneficiary.nha.gov.in using Aadhaar-based OTP verification, or get it made in person at the Ayushman Mitra desk of an empanelled hospital or at a Common Service Centre. Pre-existing conditions are covered from day one, and the benefit is portable — a cardholder can use it at any empanelled hospital anywhere in India, not just in their home state.

One caveat worth flagging for readers: those already covered under other public health schemes, such as CGHS, ECHS, or the Ayushman CAPF scheme, may need to choose between their existing cover and PM-JAY rather than holding both simultaneously — the exact opt-in rules are best confirmed on the official portal before applying.

Why This Matters for the Insurance Industry

For an industry watching healthcare cost inflation for the elderly climb every year, a government-backed, income-agnostic Rs 5 lakh cover for everyone above 70 is a meaningful shift in the risk pool insurers and TPAs have historically priced around. It also puts pressure on private senior citizen health insurance products to justify their premiums where PMJAY coverage now overlaps, particularly for lower ticket-size hospitalisations that the Rs 5 lakh limit comfortably absorbs.
